If you're parent will not be attending with you, you must have them print and sign the attached forms and bring them with you.
EVERYONE must have the Release Waiver signed by their parent if they are under 18.
If you do not have a USA membership yet, you must have the USA membership signed by your parent if you are under 18. I know it has 2011 on it, but it will still work.
